Most changes in an older person’s health start small. A little more tired one day, eating a bit less the next, confused for a few hours at the weekend. Families often sense something is wrong before anyone else, but without a structured way to write it down, those observations disappear by the next GP appointment. And dehydration, urine infections, constipation, and chest infections all announce themselves days before they become emergencies.
This log fixes that. It is built around twelve concrete signs families can actually observe (appetite, sleep, confusion, pain, skin, mood, and more) tracked across seven days. At the end of the week you can see at a glance which signs keep showing up, and take those patterns to your GP, community nurse, or pharmacist.
